Two kinds of change keep it commercial: changes of a type sold in the market anyway, and minor changes made to meet a Government requirement. A part built only to a government design is not commercial.

COTS is short for commercially available off-the-shelf. FAR 2.101 sets three tests. The item is a commercial product, it sells in substantial quantities on the open market, and it goes to the Government in the same form. Every COTS item is a commercial product. Not every commercial product is COTS. A catalog part with a small change is commercial, but not COTS.
